Forms and Publications
The forms and publications provided on this page are intended to assist in navigating the BTA’s tax appeal process. The forms and templates are fillable. Parties are not required to use these forms, though they are intended to be helpful in satisfying applicable requirements.
Notice of Appeal Forms
- BOE Appeal Form. Use this form to appeal a decision of a County Board of Equalization concerning property assessment or exemption.
- PTR Appeal Form. Use this form to appeal a reduction or denial of a property tax reduction benefit (Circuit Breaker or Disabled Veterans).
- STC Appeal Form. Use this form to appeal a final decision of the Idaho State Tax Commission (STC) concerning income, sales, use, fuels tax, and tax refunds.
Other Forms
- Settlement Form. Use this form when both parties have agreed to settle the appeal.
- Withdrawal Form. Use this form to withdraw an appeal.
- Public Records Request. Use this form to file a formal request for records.
- Auditor Transmittal Checklist (County Use Only). For use by the County Clerk/Auditor
Templates
- Notice of Appearance. Use this template to inform the BTA of a new representative.
- Certificate of Service. Use this template to certify a copy of the motion or other filing was sent to all relevant parties.
